This doctor refused to supply my husband with a one month supply of medication upon discharge from the nursing home even though he stated in a meeting with CCAC that he would do that. This doctor contributed to an UNSAFE DISCHARGE as well as FAILING TO PROVIDE THE NECESSITIES OF LIFE. This doctor has been reported to the College of Physicians and Surgeons of Ontario.
