Failed to discuss my depression or ask whether I had suicidal ideations before prescribing me a narcotic drug that is contraindicated for those symptoms. Prescribed me a higher dose than recommended to start. Failed to consider alternate explanations for my symptoms, such as bipolar disorder. Did not provide diagnostic test for bipolar disorder. The medication prescribed in contraindicated for bipolar disorder.
