i needed a form to be filled up by him. made 3 trips to the hospital and called few times and left messages and still he did not fill the form and when i was upset at him, he did not acknowledged that he did something wrong. very irresponsible and unappreciated of patients time and needs.i am even thinking of report this to the college.this sort of service is unnecessary and very poor.
